Senior Ophthalmologist | Cataract, Refractive, Squint & Paediatric Ophthalmology Specialist Dr. Chandrakant Gaonkar is a comprehensive ophthalmologist with over 40 years of experience, serving Goa since the mid-1980s. A graduate of Bombay University, he is one of the region’s earliest pioneers in phacoemulsification cataract surgery. He has performed over 15,000 cataract and refractive surgeries, including phacoemulsification, LASIK, glaucoma procedures, and squint correction. A former Associate Professor with years of teaching experience, Dr. Gaonkar is respected for his clinical excellence and compassionate, patient-focused care. He continues to lead advancements in cataract, refractive surgery, squint, and paediatric ophthalmology.

This case highlights the challenges involved in managing a traumatic membranous cataract with fusion of the anterior and posterior capsules. The primary difficulty lies in puncturing the dense, fibrotic capsule. In such cases, the goal is not to create a traditional continuous curvilinear capsulorhexis, which is often unfeasible, but rather to clear the visual axis effectively. The choice of intraocular lens (IOL) depends on the condition of the capsular bag or sulcus, with a three-piece IOL being the preferred option when support is adequate. Additionally, in cases with a central corneal scar, residual astigmatism may persist postoperatively, and should be taken into consideration during surgical planning and postoperative management. The post-op astigmatism in this case was +3.00 D x 180. BCVA was 6/18.

This surgical video demonstrates lensectomy with retropupillary iris-claw lens implantation in a young patient with a subluxed lens of more than 7 clock hours, secondary to Homocystinuria. The patient was using aphakic glasses due to significant temporal displacement of the crystalline lens. Notably, the lens exhibited a posterior tilt in the supine position, making lensectomy the preferred surgical approach.
